Foreign Student Sponsorship

F-1(Student Status) to H1-B Document List

	•		Copy of the passport.(all pages with biographical information and U.S visa stamps)
	•		Copy of I-94 (front & back) from most recent entry into the U.S.
	•		Copy of all I-20s.(front & back)
	•		Copy of practical training work authorization card, if you have full time practical training.(If you are in curricular practical training you will not have this card)
	•		Copy of all University diploma(s) and transcripts, certificates or licenses, with translations if the documents are not in english.
	•		Current contact information.(address, telephone number, etc.)
	•		Last address outside of the U.S.
	•		Copy of social security card.
	•		Copy of driver's license or state ID card.
	•		Current resume.
	•		Copy of all previous approval notices.(I-797 forms)
	•		If you have ever been arrested or charged of any offence, please provide an explanation.
	•		Detailed job description, including salary, work sites and job title **very important.
	•		Names, date of birth, place of birth, and current immigration status of your spouse and children. If you have dependents, we will need copies of their passports, I-94 cards, marriage certificates, and birth certificates.
	•		If you have any plans to travel outside the U.S, please contact our office immediately, as any travel will have an impact on your H-1B visa application.

* All copies must be readable. If you cannot clearly read all the information on the documents, then immigration cannot read them.

J-1(Student Status) to H1-B Document List

	•		Copy of the passport.(all pages with biographical information and U.S visa stamps)

	•		Copy of all IAP-66 forms.(1 for each year of program)
	•		Copy of any waiver(s) issued, if necessary, for excemption from the 2-year home residency requirement.
	•		Copy of all University diploma(s) and transcripts*, plus any certificates or licenses.
	•		Current contact information.(address, telephone number, etc.)
	•		Last address outside of the U.S.
	•		Copy of social security card.
	•		Current resume.
	•		Copy of driver's license or state ID card.
	•		Copy of all previous approval notices.(I-797 forms)

Employer Checklist

	•		Detailed job description, including salary, job title and job location.
	•		Company name and address.
	•		Name, title and mailing address of the individual signing the documents.
	•		Federal Employer I.D. Number.
	•		Year Established.
	•		Current Total Number of Employees, Current Total H-1B Employees.
	•		Company Gross or Net Annual Income.(please specify)
	•		Type of business, including a short but detailed description of the products/services offered. Please include the internet of your company if applicable.
	•		Work location of the applicant.(if different from above)
